International Travel Certificate Form

Prior to scheduling an appointment for a health certificate associated with domestic or foreign travel, please review the import-export requirements using the tools on the USDA website. Also, if flying, contact the airline for specific requirements regarding caging and health certificate timing.

As part of our efforts to provide you with the most comprehensive New York City pet healthcare services, our veterinarians are USDA certified and can provide international pet health certificates, which are required when traveling with your pet outside of the United States.

Many countries have their own pet health certificates and travel requirements, available here www.aphis.usda.gov/aphis/pet-travel. If you are traveling to a country without an approved health certificate, please Review USDA’s Pet Travel Website for more information.

The International Health Certificate should be issued by one of our USDA-certified veterinarians as near to the travel date as possible, but not more than 10-14 days before travel depending on the location.

Along with an international health certificate, there may be additional requirements for traveling abroad. Please take some time to familiarize yourself with the international pet travel requirements specific to your destination. If you have any questions, our staff is happy to help.

PLEASE FILL OUT AND RETURN TO US AS SOON AS POSSIBLE IN ORDER TO START RESEARCHING FOR DESTINATION’S REQUIREMENTS
